What matters most to you when you think about your health changing?

Advance care planning is a process of thinking and talking about your values and goals and what your preferences are for your current and future healthcare. It helps you understand what the future might hold, and to say what health care you would or would not want, its more than just your end-of-life care. Updating your advance care plan on a yearly basis ensures information is correct and up-to-date.


Amyes Road Medical Centre is now offering free Advance Care Plan appointments with our Nursing team.

You can download a self editable electronic version of the Advance Care Planning Booklet and email to admin@amyesrd.pegasus.net.nz  or call reception on 03 349 8263 to get started.


An Advance Care Plan is carried out in 5 steps:

1) Thinking about

Think about whats important to you and what gives your life meaning.

2) Talking about

Talk about your decisions then others will know what is important to you and what you would like to happen, especially if you’re not able to speak for yourself.

3) Planning for

When you know what’s important to you, and what you want others to be clear about, we write it down in the ACP booklet. This means it can be no argument about your wishes when you can’t speak for yourself. You can get started by accessing the booklets here. Bring a copy with you to your appointment.

4) Sharing

This document can be shared with friends and family, a copy kept at Casebrook Surgery to be accessed by your clinical team, and with Te Whatu Ora (Health New Zealand) so if you’re ever in hospital the team will know your wishes even if you can not share them yourself.

5) Reviewing

We will check in  every year to ensure your wishes are still up-to-date and accurate.
